Forms and Documents

Below you will find some forms that are necessary if your:

Student has Asthma...doctor and parent/guardian must complete the Asthma Action Plan (please bring the inhaler in the box from the pharmacy with the child's name on it - please check the expiration date) *PLEASE NOTE - Due to COVID-19, nebulizer treatments will NOT be administered in school until further notice.

Student has an allergy requiring an epi-pen or Benadryl...doctor and parent/guardian must complete the Allergy Action Plan (please bring epi-ipen and/or Benadryl to school in the box from the pharmacy with the child's name on it - please check expiration date).

Student wants to participate in school sports...doctor and parent/guardian must complete a PPE (Preparticipation Physical Evaluation) within 365 days of the first day of try-outs/participation (parent/guardian completes page 1 - and page 2, if applicable; doctor completes pages 3 and 4).  If PPE was completed 90 days or more prior to the first day of try-outs, then a Health History Update Questionnaire must be completed by the parent/guardian.  Parent and student must also complete the After School Permission Slip.

Student requires medication in school...doctor and parent must complete the Medication Permission Form - required for all over the counter medication also (please bring the medication in its original container - unless regular strength Tylenol or ibuprofen, which is supplied by the school).

Student has a seizure history...doctor and parent must complete the Seizure Action Plan.

Attention Parents of 6th Grade Students

The State of New Jersey has immunization requirements for all incoming 6th grade students.  As of September 1st of their 6th grade year, students must have documentation of a Tdap vaccine (tetanus, diphtheria, acelluar pertussis given no earlier than their 10th birthday) and a meningococcal vaccine.  Please send in documentation of these vaccinations as your child receives them, even over the summer.  Forms are available below.  If you have any questions, please contact your pediatrician or me at 609-267-2722 or kosullivan@westamptonschools.org.

Physicals by School Doctor

The school doctor will be at school twice a year to perform free sports physicals for those students who would like to participate in a school sport but have been unable to obtain a physical by their family doctor. A health history form and a permission slip must be completed by the parent/guardian before the physical can be done.

It is also important that students receive a medical examination at least once during each developmental stage, at early childhood (pre-school through grade three), pre-adolescence (grade four through six) and adolescence (grades seven through 12).

Health Advisory

The CDC recommends three basic precautions you can take against influenza.

(1) TAKE TIME TO GET A FLU VACCINE

The CDC recommends taking time to get the influenza vaccine as your first precaution against influenza. Unfortunately, explains the CDC, the influenza virus mutates constantly, so you must get vaccinated every year. The vaccine is available as an intramuscular injection (shot) or an intranasal spray. In either form, the vaccine can reduce your risk of getting influenza by up to 90%.

(2) TAKE EVERYDAY PREVENTIVE ACTIONS

Your second-best defense against influenza is to take everyday preventive actions. These consist of frequent handwashing, using tissues when available or sneeze into your elbow and staying home from school when you are symptomatic. Students with a fever must remain home until fever-free for 24 hours.

(3) TAKE FLU ANTIVIRAL DRUGS (if your doctor recommends them)

Antiviral drugs are a precaution appropriate for some people. Speak with your doctor to determine if these drugs are appropriate for you.

New Jersey has made a commitment to provide universal health insurance to children and provide affordable options for parents/guardians. NJ Family Care provides free or low cost insurance for chldren and thier parents/guardians. Parents or guardians who earn too much to qualify for NJ Family Care can purchase health insurance for their children at reasonable rates through another program called the NJ Family Care Advantage program, which is administered by Horizon NJ Health.
