Below you will find some forms that are necessary if your:

Student has Asthma...doctor and parent/guardian must complete the Asthma Action Plan (please bring the inhaler in the box from the pharmacy with the child's name on it - please check the expiration date).

Student has an allergy requiring an epi-pen or Benadryl...doctor and parent/guardian must complete the Allergy Action Plan (please bring epi-ipen and/or Benadryl to school in the box from the pharmacy with the child's name on it - please check expiration date).

Student wants to participate in school sports... After your doctor completes a physical on your student, they must complete a Medical Eligibility Form, which then must be turned into the school. These forms provide clearance to participate for 365 days from the date of completion. If a try-out or start of an activity is after 90 days of the clearance, a Health History Update Questionnaire must be completed by the parent/guardian be eligible to participate. Parent and student must also complete the After School Permission Slip.

Student requires medication in school... Your must complete a Medication Permission Form, which then must be turned into the school. Medication is to be brought to school by the parent/guardian in the original container.

Student has a seizure history...doctor and parent must complete the Seizure Action Plan.
