Attention Parents of Incoming 6th Grade Students,
The Department of Health is requiring that documentation of the Tdap and Meningitis vaccines MUST be submitted by September 1, 2021 or documentation of the scheduled appointment in order to start 6th grade. Any student who turns 11 years old in September, must submit documentation of the scheduled appointment also. Documentation may be mailed, faxed (609-702-9017) or emailed to pherriman@westamptonschools.org.
